Evelyn Ann Forrest, age 76, transitioned to her Heavenly Home May 6, 2023. Preceded in death by parents, Andrew and Evelyn Johnson Jefferson; brothers, brothers, Thomas, Andrew, and Charles (Rashunda). Survived by son, Richard Duckett, Jr; Siblings, Betty Jean and Michelle.

Home-going Celebration 10AM Saturday, May 20, 2023 in the Chapel of SMOOT FUNERAL SERVICES, 4019 E. Livingston Ave., Columbus, Ohio. Family will receive friends 9AM until time of service.
